Our substance abuse counselors and staff will help your loved one create a long-term recovery plan for life after treatment, and we also encourage alumni and their families to participate in support group sessions.

Individual and group therapy sessions are offered for alumni as needed. Family members are welcome to attended scheduled sessions whenever they would like.

Following treatment, our addiction specialists will assist clients by recommending halfway housing, family therapy, meeting locations, counselors, extended care facilities, and much more. Former clients and their families are always welcome at Destination Hope. We also firmly believe in maintaining regular communication with our alumni.

What is Aftercare?

Aftercare is a continuation of counseling and support for life after rehab. Recovery is a lifelong process and aftercare can help rehab graduates continue their journey and live a healthy, sober life. Aftercare can aid in preventing a relapse and expand the coping strategies that will be learned during rehabilitation.

Aftercare often involves less contact with treatment personnel, which allows each woman to enter real-life situations with self-sufficiency and individual determination in maintaining her sobriety.

When Does Rehab Aftercare Start?

Rehab aftercare programs can begin as soon as you graduate from your drug or alcoholism treatment program.

Aftercare is especially important for those who completed inpatient programs. During inpatient addiction treatment, patients receive constant care and their days are filled with activities. The transition into everyday life can be wrought with temptation and risky environments. Outpatient alcohol and drug treatment can help you shift into a new sober life with new habits and ways of thinking that will reduce your risk of relapse.

Types of Aftercare Programs

Following the completion of a rehabilitation program, our staff can help patients in finding aftercare programs. There are different types of aftercare programs, including:

  • Outpatient treatment: Our intermediate intensive outpatient programs are ideal for those who wish to continue treatment on a part-time basis.
  • 12-Step programs: During 12-step meetings, patients will receive and provide support and encouragement from those also in recovery.
  • Sober living homes: Sober living homes are group homes for those in recovery. Women will have the opportunity to continue in long-term recovery while still working, going to school, and maintaining daily responsibilities.

After treatment, we help our clients create an aftercare plan, which can include recommendations for halfway houses, family therapy, 12-step meeting locations, counselors, psychiatrists, and extended care facilities.

Goals of Aftercare

Each aftercare plan is personalized to individual needs, however, there are some common goals of an aftercare program for addiction and substance abuse.

  • Helping patients make healthy choices about their lifestyle, activities, and relationships
  • Reinforcing skills for coping with stress and emotions
  • Teaching you to identify triggers in order to prevent a relapse
  • Teaching you to minimize the damage of a relapse
  • Providing access to a support group or individuals who can help you through the recovery process

Aftercare is an essential part of long term addiction recovery, and setting goals is equally as important to your sobriety.

Aftercare programs provide an extra level of accountability to prevent a relapse. Some of the benefits of addiction aftercare programs include:

The Destination Hope discharge team and case workers work with a select network of reputable sober living or transitional living homes for both men and women in Florida and other parts of the country. We are meticulous about the sober living homes we recommend, and they must meet specific accreditation and ethical standards according to our guidelines.

Recovery is a life-long process that does not end after treatment.  It is essential that patients and families find a supportive environment that will help guide them through life after rehab.

After completing a drug treatment plan, aftercare should be the next step in your rehabilitation. Aftercare provides a continuation of counseling and support once a rehab program is successfully completed. By continuing with an aftercare program, you reduce the risk of a relapse.

The Relapse Process

Relapse is often linked to three different roots, emotional struggles, unhealthy patterns, and external situations. Some factors that can contribute to a relapse include:

  • Being unable to manage stress or negative emotions
  • Interpersonal conflicts with family and/or friends
  • Failing to follow the recommendations of counselors
  • Negative thinking (i.e., feeling deprived or resentful)
  • Low motivation

Cravings for old behaviors and the belief that using once again will make you feel better are also common.

When those cravings begin, or any other factors that contribute to a relapse occur, it is important to have a plan in place. Though a person’s ability to succeed plays a role in overcoming a substance abuse disorder, rehab aftercare can help you build your abilities to overcome situations that can put you at risk of a relapse.

Creating a Relapse Prevention Plan

Preventing a relapse will require a plan tailored to meet the needs of each individual person. For some, this plan is a lifelong health plan that will include individual counseling, group therapy, ongoing addiction education, recreational outings, and more.

Another part of your plan should be creating a support group that will help you commit to living a sober life. Your sober support system can include support groups and organizations, counselors, and sober friends and family.

A sponsor is another person you may want to integrate into your sober support system. A sponsor is someone who has considerably more time in sobriety than you. Sponsors offer the freedom to call them at any time when you may need extra support. Your sponsor should be someone you are open and comfortable with that you look up to as a sober mentor.

Support after Treatment

Destination Hope is proud to continue supporting clients long after treatment. Our rehab center offers lifetime support and we encourage alumni and their families to attend and engage in aftercare programs.

Following the completion of a treatment program, our counselors will work with you to create a relapse prevention plan that will help you succeed in continuing to abstain from drugs and alcohol.
